Rachel Andrew is the Director of Clinical Programs at Premera Blue Cross where she leads a multidisciplinary case management, care transition management, disease management and wellness team. She is the co-author of the book The Integrated Case Management Manual: Value-Based Assistance for Complex Medical and Behavioral Health Patients, Second Edition (2018). Her prior experience includes over 25 years university teaching in behavioral health and education programs and over 10 years in community mental health. Rachel has been the director of four clinics and has facilitated grass root community projects focused on improving the well-being of the community as well as providing mental health therapy in medical and psychiatric hospitals, schools and other settings. She holds certifications in minority mental health issues, integrated case management, LEAN, creative problem-solving and is a certified case manager and child mental health specialist. e: rachel.andrew@premera.com

Q: What does Person-Centered Care mean to you?

RA: Person-centered care begins by listening and meeting the person where they are. It includes taking a whole person (e.g. physical, emotional, social), relationship-based approach to address the person’s goals. It is built upon respect for the person, leveraging their strengths and empowering them to make informed decisions to improve their lives. The clinician is a guide, walking along side the individual on their journey to health improvement.

Q. Why should individuals attend this year’s conference/your presentation?

RA: TCOM conference participants will learn how to leverage data to improve their service effectiveness and demonstrate value – quality, experience and cost. The presentation on Integrated Case Management will introduce participants to a new tool, INTERMED Complexity Assessment Grid (ICM-CAG). The ICM-CAG is aligned with TCOM tools and measures clinical and non-clinical barriers to health improvement in the biological, psychological, social and health system domains. This presentation will make the case for an integrated and relationship-based, whole person approach to removing a person’s barriers to heath improvement. It will include change management strategies, implementation tips and outcomes.

Q: What drew you to attend this year?

RA: The focus on a person-centered approach, measuring outcomes and the opportunity to network with colleagues in the field.

Q: Why did you choose to present on this specific topic?

RA: Evidence supports a strong link between physical and behavioral health conditions. The lack of integration has a significant negative impact on individual’s lives. I have witnessed the effectiveness of the Integrated Case Management model in addressing individual’s barriers to health improvement and would like to share these findings with others. My passion is developing clinicians and improving the quality of care and experience individuals have in the healthcare system. My goal is to enhance clinician’s skills and advance the case management field.

Integrated Case Management: Value-based assistance for complex medical and behavioral health patients:

This presentation provides participants with an overview of Value-Based Integrated Case management (VB-ICM) utilizing the INTERMED Complexity Assessment Grid outcome tool. VB-ICM model begins by listening, meeting patients where they are at and delivering value as determine by the patent. VB-ICM managers are cross-disciplinary trained, providing patients with a single-point-of-contact for all their health care needs. VB-ICM’s patient-centric approach focuses on removing patient’s barriers and support patients to make the behavioral changes and healthcare decisions that align with their values. Measuring and reporting the patient’s clinical and functional, as well as program effectiveness goals, is the core of this model.
