Because TCOM Tools are designed for information integration, the assessment provides a comprehensive summary of the person’s story. This information is then used to guide the development of an action plan for the child, youth, individual, or family.
The next step is to work collaboratively with the child, youth, individual, or family to develop a shared understanding of the needs they want to address and the strengths they want to build or use. Once this shared understanding is established, you can co-create a vision of what life could look like when their goals are achieved.
With a clear understanding and shared vision in place, you will then work together to develop specific action steps to reach those goals.
How can I guide people in creating action plans?
Guiding people in creating action plans starts with collaboration and shared understanding. Begin by reviewing the assessment together and ensuring the child, youth, individual, or family feels heard and understood. Use the information from the TCOM Tool to identify priority needs and meaningful strengths.
Next, support them in identifying which needs they want to address first and which strengths they can build on or use to support progress. From there, help co-create a shared vision of what success looks like in their daily life.
Once priorities and vision are clear, work together to develop specific, achievable action steps. Each step should be clear, measurable, and connected to the identified needs and strengths. Be sure to identify who is responsible for each step and establish a realistic timeline.
Finally, emphasize that action planning is an ongoing process. Regularly revisit the plan together, celebrate progress, and adjust steps as needed to stay aligned with their goals and evolving needs.
