The 22nd annual TCOM Conference is a three-day gathering for a broad audience engaged with or learning about TCOM, building skills, connections, and insights with TCOM framework. We invite people with lived experience, academics, and the individuals and teams who work in government, nonprofit, direct service, data science, policy, community programs, or other settings to this dynamic exchange.

Submitting a Proposal? Here’s a Checklist of What You’ll Need:

Breakout session requirements:
  • Abstract of presentation (max. 500 words) 
  • Brief description of your session (max. 100 words) 
  • 3-4 learning objectives 
  • Target audience(s) for the presentation 
  • Contact information for all presenters

 

 

Poster session requirements:
  • Poster title
  • Poster abstract
  • Poster presenter and co-authors’ names & affiliations
  • Poster lead contact email address
  • 3-4 learning objectives

What is the Theme This Year?

Ever Forward: Growing Community, Cultivating Outcomes 

This year’s theme is grounded in the belief that our work and outcomes cannot exist in silos. True collaboration, always requiring practice, means there is a confluence of ideas, perspectives, intentions, partnership, accountability, and a shared vision. Growing community means deepening our relationship with affected communities, including youth and families, who are not only welcome in this space, but essential partners to shape outcomes. 

Our ambition is that this TCOM Conference, like those before it, plants a seed of community that bridges disciplinary and professional divides, and connects people across roles and experiences, just as TCOM as an approach works to overcome the institutional barriers that obstruct the proper provision of care. We view the community of policymakers, direct care workers, analysts, advocates, trainers, community leaders, and cross-system partners as collaborative. The conference is a space where this collaborative can strengthen itself and engage in a collective effort to improve person-centered care.
