What can TCOM tools do for my agency/jurisdiction?
TCOM tools are designed to help human service agencies shift from process-driven workflows to person-centered, outcome-oriented care. They move the focus from merely tracking compliance with paperwork and completion of hours of care towards identifying meaningful change in the lives of those served. Agencies can better measure the effectiveness of their services, helping them understand what works best and where improvements are needed. Each tool works in a separate situation but is guided by the same underlying philosophy, meaning that their implementation gives coherence to an agency’s approach towards evaluating the progress of those under care.
How do TCOM tools give voice and choice to people?
TCOM tools are communimetric measures – they are based on a theory of measurement as communication, meaning that the tools measure the reality of those receiving care and their families as they themselves perceive it. This makes them culturally responsive to the lived experiences of each individual rather than treating culture as a constant. Since the tools are carried out in collaboration with people in care and their families, the assessment reflects their goals and priorities, empowering them to make relevant decisions in the design and execution of their care plans.
How can TCOM tools reflect the unique circumstances of my agency/jurisdiction and its population?
Various jurisdictions have adopted uses of the TCOM tools that vary to some extent from its standardized form. TCOM jurisdiction-specific training is offered so that your agency can meet its own unique needs and values in its implementation of the TCOM tools.
