The Center for Innovation in Population Health at the College of Public Health at University of Kentucky is hosting a 3-day international 19th Annual TCOM Conference. The presentations at this conference reflect the use of Transformational Collaborative Outcomes Management (TCOM) and its tools. TCOM is an approach grounded in a philosophy of a single shared vision–helping people achieve their health and wellness goals as they navigate healthcare, child welfare, juvenile justice, behavioral health, education, and other complex systems.

The Conference Learning Objectives

Upon completion of this activity, participants will be able to:

  • Discuss how to communicate to families/individuals, and other professionals in the field through a common language.
  • Review the TCOM framework and how it guides the development of a shared vision for the people we serve, providers and systems.
  • Describe how to implement TCOM tools effectively.
  • Use person-centered data to show change and outcomes.
